Health Consultation Programs (HCPs) are free programs offered to our contracting towns where a nurse will meet with individual adult residents to confidentially discuss health issues/concerns along with preventative measures for better health. A health history along with blood pressure measurement, and height/weight measurements are performed during the meeting. Referrals to Bergen County as well as outside resources are available. Individualized counseling, education and follow up is conducted.
